Kybella® — Deoxycholic acid. FDA-approved for moderate to severe convexity or fullness associated with submental fat in adults; the label permits up to six treatments, at least one month apart. Injection anywhere else on the body is an off-label use, which means the FDA has not reviewed that particular use: the drug is the same, the indication has not been through approval.
Deoxycholic acid — A bile acid the body already makes to break down dietary fat. It disrupts fat-cell membranes on contact and is not selective about which cells it reaches, which is why the depth and placement of each injection decide the result more than the volume given.
Topical and injected local anesthetic — Lidocaine, with or without epinephrine, plus ice. Comfort during a Kybella® session comes from a combination rather than a single agent, because the burning is produced by the drug acting on the fat and not by the needle.
Compounded lipolytic injections — Phosphatidylcholine and similar compounded fat-dissolving mixtures are not FDA-approved for fat reduction, and are not used here.

The serious risks are uncommon. Kybella® off-target can injure the marginal mandibular nerve, showing as an uneven smile, usually temporary. Into gland, muscle or skin, it can cause ulceration. Difficulty swallowing has also been reported.
